Supplies needed:

  1. None needed...

Procedure:

  1. Knock, greet resident by name and introduce self and title. Explain intended procedure and ask permission.
  2. Ask patient if they need to use restroom first
  3. Excuse self to wash hands, give call light
  4. Knock, greet resident by name
  5. Pull curtain closed
  6. Lower bed to lowest setting (you can raise the head of the bed at this time if necessary)
  7. Assist patient to sit up and dangle
  8. Verbalize not dizzy? and feel o.k.
  9. assist patient to put on non-skid footwear, sweater if cool etc.
  10. help to stand and walk to scale
  11. Zero the scale
  12. get weight and height measurements (w/in 1 inch of CSO and 2# of CSO)
  13. assist onto/ off of scale
  14. return patient to bed, remove footwear
  15. Remember pretty beds, lower beds, call bell, check comfort, open curtain
  16. Properly record (pt. name, date, time, your initials)
  17. wash hands

Testing Checklist:

  • Did candidate zero scale before measuring?
  • provide assistance walking, and on/off scale platform?
  • Record height w/in 1 of CSO?
  • Record weight w/in 2# of CSO?
  • Wash hands before and after?
