Supplies needed:

  1. food tray
  2. 2 choices of food, spoon
  3. Full glass water (240ml) with straw
  4. Shirt protector
  5. napkin or facial washcloth
  6. towel

Procedure:

  1. Knock, greet resident by name and introduce self and title. Explain intended procedure and ask permission.
  2. Excuse self to wash hands and gather supplies. Give call button.
  3. Simulate hand wash and gather supplies
  4. Knock, greet resident by name
  5. Set up table, cover with towel and lay supplies down
  6. CLOSE CURTAIN FOR PRIVACY
  7. Offer choices
  8. Raise bed to High Fowler's position
  9. Place shirt protector
  10. Offer washcloth to wash hands before eating
  11. Begin w/ water ÎíÎñ every 3 bites: water & wipe
  12. Remove shirt protect, make sure pt is clean, offer wash cloth to wash hands
  13. Clean up table
  14. Leave in sitting position for 30 minutes to avoid choking hazards (verbalize this to patient)
  15. Pretty beds, lower beds, call bell, check comfort, open curtain
  16. Take care of supplies
  17. Wash hands
  18. Record intake

Testing Checklist:

  • Did the candidate offer and assist to wash hands before feeding?
  • Sit at eye level?
  • Apply shirt protect?
  • Offer fluid before food?
  • And then between 3 bites of food?
  • Check to see if theyÎéÎ÷ve swallowed before offering next bite?
  • Offer encouragement in eating & fluid intake?
  • Converse?
  • Clean and dry face?
  • Clean area. Record intake Wash hands before and after?
